I think it's someone the prisoners have been in communication with on the outside. Probably whomever was watching Carol - and maybe took her? I don't think she is dead. They aren't going to do a silent kill on her while showing the death of T-Dog and Lori.
Pretty sure the person watching Carol was the "escaped" prisoner. I agree that Carol is likely still alive but confusing considering the had a filled grave (ie Daryl putting the flower on how grave).
