Since patch 4 I have played several games of 2k with no issues or occurrences of ce-34878-0.
That's good maybe it is an issue with the game and not the system then.
From what I have read that code covers many issues.

Glad to finally have Pulse support.
I'm probably going to pass on the new headset. I have both the Pulse and the Pulse Elite, so I really don't need it.
However, the Elite headset is a bit uncomfortable for me. The audio profiles are great though, so I still use it for short burst game play. The regular Pulse is super comfy, and this looks like that design + the profiles.... So, yea, maybe.
Why is it every time I try to do something on PS4, there's something wrong? I download Outlast to play through. "This game can only be launched by the person who owns the content." Which is me. I try to restore licenses about 20 times and just get errors. I try to go to the store to re-download...errors. Okay, I'll just play Battlefield while they sort their stuff out. "Battlefield servers are down."
PSN users (PS3 & PS4) may want to change their password. My account was hacked/compromised this morning and a Twitter search shows that others are having the same issue.
Sony was able to correct the issue for me in a matter of minutes, FWIW. The guy(s) responsible were *nice* enough to brag about it to me on Twitter. As far as I can tell, they didn't do anything but they may not have had time. One Twitter user is saying that their GTA Online cash was wiped out. If they did that to me, I haven't checked but I know they didn't get rich.
I do not think it is a widespread issue, but wanted to give a heads-up. I'm not sure why they thought I was cute enough for it.
- Unrelated, but many users are having an issue with the PSN Store. They are seeing a message saying it is unavailable in their country.

Also, I'm not getting any sound from either Pulse or Elite headsets.
This 3 months since launch has been a disaster network wise. It was one thing for them to pull this garbage when it was free to play online, but it's unacceptable for this to happen as often as it does on a pay to play service. I understand Sony saw all the money Microsoft was making off of Xbox Live, but Microsoft has the infrastructure to provide a service that is quality enough to be a paid service, while Sony clearly isn't there yet.
It seems like every time time I hop on something is wrong with it. Meanwhile, I haven't had a single online issue with the Xbox One, even on launch night when everyone was pounding the servers.
