Goals from Diego Costa and Lucas Piazon lifted Chelsea to a 2-1 win over West Ham United on Sunday as the Blues continue to zero-in on the Premier League title. Chelsea need just 2 points from their final 4 league matches to earn the 5th top-flight domestic league title in club history.
The title looks likely to be earned at Stamford Bridge on Wednesday night when Chelsea (24-6-4) host 18-placed Cardiff City (6-9-19). Maximum points over the Bluebirds would secure the title and a double for the Blues who have already lifted the Capital One Cup this season. Chelsea were 4-0 winners in Cardiff on November 21.

Despite conceding twice, including once in the first minute, Chelsea secured the Premier League title with a convincing 7-2 win over Cardiff City at Stamford Bridge on Wednesday night. The league title, the Blues first since 2009-10, secures a domestic double for the West Londoners who have already won the Capital One Cup. With 3 league matches remaining, Chelsea can move their focus to a pair of cup finals later this month.
On Saturday Chelsea (25-6-4) will visit Anfield to play the red half of Merseyside. Liverpool (17-10-8) are currently clinging to the last Champions League spot but are not assured of a spot in Europe as they lead 6th placed Everton by just 3 points.

Having already secured the Premier League title, Chelsea traveled to Merseyside on Saturday and came away 1-0 winners behind an 84th minute goal from Marco Reus and a clean sheet that was kept by Thibaut Courtois.
The Blues announced on Sunday that they have added Brazilian 15-year-old Andriy Mielitz to their youth squad and that they have extended the contract of Diego Costa. The Spain international Costa, 27, won the Premier League's Golden Boot in 2014-15 with 22 goals and this year is the league leader with 16. The striker has accounted for 32 goals across all competitions in 42 appearances this campaign. Costa's new contract ties him to the club through the 2019-20 season and will see his pay jump to 190k pounds per week making him the second-highest paid player in the squad behind Marco Reus (195k).
Chelsea (26-6-4) will host 16th-placed Stoke City (9-13-14) at Stamford Bridge on Wednesday night, ahead of Saturday's FA Cup Final with London rivals Crystal Palace at Wembley Stadium.

In advance of Saturday's FA Cup Final, Chelsea were 4-1 winners at Stamford Bridge over Stoke City behind a first half hat-trick from Spanish international Diego Costa who leads the Premier League with 19 goals on the season.
Having already secured the Premier League title as well as having won the League Cup, Chelsea will attempt to win a domestic treble this weekend when the Blues travel a short distance to Wembley to face League Championship side Crystal Palace in a London Derby that will decide the 2016 winners of the English Football Association Challenge Cup.

Chelsea secured a domestic treble on Saturday at Wembley Stadium, winning the 8th FA Cup in club history with a 1-0 result over South London rivals Crystal Palace. The Blues are FA Cup winners for the first time since getting a 2-1 result over Liverpool in the 2012 Final and add the illustrious silverware to the Capital One Cup and Premier League title they have already captured this season. The West Londoners will play for an unprecedented quadruple on Saturday May 28th when they face Spanish giants Barcelona in the final of the UEFA Champions League.
Chelsea's victory did come at a high cost, however. Marco Reus (Femur Contusion) and Diego Costa (Sprained Ankle) were both lost to first half injuries. Boss Jose Mourinho is hopeful that the duo can make a return for the Champions League Final but has ruled each out for next weekend's Premier League finale. On a bright note, Mourinho added that Ramires (Sprained Knee) and Eden Hazard (Broken Ankle) have returned to training and that each of them could not be ruled out of featuring later this month against Barcelona.
Chelsea (27-6-4) will go through the motions one more time on Sunday, before being presented with the Premier League trophy. The Blues will host 18th-placed Leicester City (5-13-19). The The Foxes are 3 points from safety and are -6 on goal difference to 17th-placed West Ham, making Leicester an all-but-certain candidate for relegation with Cardiff City and Norwich City.
Swansea City (winners of The Football League Championship) and Crystal Palace (2nd in the League Championship) have attained promotion to the Premier League for 2016-17. They will be joined by the winners of the Promotion Playoff which features Burnley, Nottingham Forest, Blackburn Rovers and Wigan Athletic.
In the Premier League, Chelsea, Spurs and Manchester City have secured Champions League spots for next season, with the final spot up for grabs between Liverpool (64 points) and Southampton (62 points). Everton, Manchester United and Arsenal each have a chance to secure a Europa League spot, pending this weekend's results. Southampton visit Aston Villa, while Liverpool visit Watford on Championship Sunday.
