I've found the way to do this using old school auto-pass coach mode.
Switch to the pitch man then push/click down on the left analog stick and that locks the icon to that player. Now snap the ball and the QB will then carry out the option as he should.
Now don't get your hopes up, the game in general still doesn't run option plays very well but in the right situation against the right defense you can get some good gains out of the option plays both on QB keepers and pitches.
