Regarding your concerns about fatigue in your players.
I've got all my sub in set 85 and sub out set at 84 so usually I'll have a player go out for 1 or 2 plays and come right back in. It keeps everyone pretty fresh and they don't miss many plays.
The one thing you might want to do is set those values down just a bit more for HB so that the starter doesn't get stuck out of the game late.
Doing this also gets more players in the game so your backups get some playing time to help with progression and hopefully not get starters injured staying in the game so long and getting so tired.
